Vasospasm IS a thing, but very uncommon. I had a 14 year-old girl who had POTS and weekly infusions (she refused any other vein but the same one). After 6 months, she had an issue for a few weeks where the IV would run fast and then slow (one liter over 45-60 minutes on gravity) even though she was diligent about keeping her arm still. My last cannulation to that vein was easily successful with flashback and advanced cannula easily, but absolutely no blood return when the needle was removed. No swelling, no bruising and connected fluids to flush line and it would not flow at all. No infiltration. Scarring of the vein would have been at the insertion site, so not that. Her mom allowed her to refuse the IVs and after 2 weeks her migraines came back and she agreed to using the other arm.

That looks like a very large gauge needle. Why do nurses insist on doing this? Many fewer veins would blow if a smaller gauge was used. As a patient, I have had hundreds of veins blow due to nurses not listening to me. As patients we know our own veins better than any nurse. Please listen to your patients.

Is the introducer needle partially withdrawn after seeing blood flash back only ? Or after both advancing the introducer needle for at least 1 cm coupled with seeing a blood flashback ? Today, I couldn't advance the catheter after partially withdrawing the needle despite seeing blood flashback 😅

I struggle most with advancing the tube in any cannula bigger than 22G. Confidence was really shot for a while.

Do you have a video of a full check of this machine? I am not familiar with the GE machine as I regularly use the Drager ZUes and Tiro. I need to learn how to use this one for work as agency nursing . most other hospitals use this machine. I love your videos . really informative and clear.

See the thing is, this patient's vein IS visible, so it is kinda easy if you know how to do it, but PLEASE do a video of how to do it when there is NO visible veins (especially in elders and cancer patients it is very hard to find veins)

highly recommend to shave the skin before applying the plaster. it hurts when removing.

For the advanced level case of the pancreatitis patient with a beta blocker, would stopping it rapidly not cause reflexive issues? How is this managed?

I remember having this done in my upper arm for a midline. Was pretty interesting to watch. The nurses were excited becuase they said I have large veins and it was gonna be easy lol

Fantastic presentation, thank you very much - and good luck in your final exam! 3 remarks: a) given that it seems to be a slow bleed, you could do a contrast CT first to determine bleeding location and maybe it´s amenable to a interventional radiological procedure like coiling (before doing a very invasive procedure like laparotomy or thoracotomy) and b) does the initial X-ray actually show a situs inversus totalis or was that just something to throw off the candidate...? c) I´d also place a indwelling urinary catheter and have at least hourly a look at adequate urine output
